Anyone can refer someone for home palliative care or counselling services, though it is best to have a patient's existing doctor or nurse request that he or she be seen.
However, before a palliative care doctor can see a patient, the patient must ensure that his or her current doctor is aware of the referral being made.
Referrals for inpatients staying at Mount Sinai Hospital must be requested by hospital staff.

In order for an individual to be seen by a home palliative care doctor, a referral form must be completed and sent to our office.
A referral form may be downloaded or obtained from our office. To obtain a referral form or to find out more about our programs,
please call the office at the number listed above.
Once a completed referral form has been sent to our office, a palliative care doctor will call the patient shortly before the doctor is able to visit. To provide optimum palliative care, a referral form should be completed and sent to us as early on as possible. There may be a short wait before a palliative care doctor can visit the individual in the home .
